At OCSF, we know your appointment is important, and being prepared can help you get the most out of your visit. Having the right information ready and knowing what to ask your doctor can help make your appointment more productive.

Appointment Information

Whether you’re visiting OCSF for the first time or returning for follow-up care, preparing ahead of time can help make your appointment as easy and stress-free as possible.

Confirm Your Appointment Date & Time
Double-check the date and time of your appointment before your visit. You may need to complete paperwork or provide identification and insurance information before seeing your doctor, so please allow additional time before your scheduled appointment if needed.

Complete Your Phreesia Pre-Visit Intake
For your convenience, OCSF uses Phreesia for patient intake. Please complete any requested pre-visit information and forms through Phreesia before arriving for your appointment.

Confirm Your OCSF Location
OCSF has multiple office locations throughout South Florida. Be sure to confirm which location your appointment is scheduled at and review the address before leaving.

Plan Ahead for Travel & Parking
Allow extra time for traffic, road construction, detours, parking, or other unexpected delays. Planning ahead can help ensure you arrive on time and have a smooth start to your visit.

What Should I Bring?

  • Current photo ID

  • Current insurance card(s)

  • Insurance co-payment, co-insurance, and/or deductible information, if applicable

  • Insurance referral, if applicable

  • Please bring or have your provider send any medical records, imaging, and lab results related to your condition before your appointment.

  • Personal Documentation Of Current Injury Or Condition

  • List of Current Medications

  • Medical History

  • List of Questions or Concerns

  • A Family Member or Friend
